Objectives: To characterize the metabolic activity of Wilms tumor using 18F-FDG PET CT.
Methods: 39 scans were performed in 17 pts (patients) with proven Wilms tumor(ages 4-27 yrs,median 10;8 boys,9 girls). 2 pts(2 scans) were studied at the time of diagnosis or following initial surgery. 14 pts,including one studied at diagnosis(30 scans), were studied at the time of suspected relapse,progressive disease and/or for monitoring of therapy. 2 pts with history of Wilms tumors(7 scans) were studied for secondary neoplasms. After a 4 hour or overnight fast,pts were injected with 0.15mCi/kg FDG intravenously. One hour later whole body PET and CT scans were performed.Images were reviewed by 2 nuclear medicine physicians both qualitatively and semiquantitaively.
Results: In the 15 pts with Wilms tumor only, there were 26 scans with areas of abnormal uptake consistent with metabolically active tumors. 7 pts(23 scans)pulmonary metastases, 5 pts(7 scans) hepatic metastases,8 pts(8 scans)regional nodal involvements,2 pts(2 scans)bone metastases,1 pt(1 scan) chestwall involvement,1 pt(1 scan) pancreatic metastasis and 2 pts(2 scans) pelvic soft tissue involvements. 2 of 7 pts with lung metastases had variable uptakes; lung lesions less than 10~15 mm were not consistently visualized on PET scan; 1 pt with liver metastasis showed no uptake on PET scan after treatment(size decreased from 4.8 to 2.4 cm).
Conclusions: Most Wilms tumors concentrate FDG. However,small pulmonary metastases and liver metastases may be better visualized with CT. FDG PET CT appears useful for defining the extent of involvement and assess response to treatment.
