Objectives: Measuring patients blood glucose (BG) levels prior to PET/CT with 18F-FDG is of paramount importance in insuring an optimal scan. However, some PET/CT institutions measure BG levels in diabetics (D) only. The purpose of this study was to evaluate the prevalence of abnormal BG level among both D and non diabetics (ND) undergoing FDG PET/CT examination.
Methods: Medical records including technologists notes were retrospectively reviewed for a total of 1465 PET/CT exams and a log was kept to record BG level measured at the time of FDG injection as well if patients were known D or ND on the date of the PET exam. Using a normal BG range of 70-140 mg/dl, each BG measurement was categorized as normal or abnormal BG level (<70 or >140) among D or ND undergoing a PET/CT exam.
Results: Of the 1465 patients, 1330 (90.8%) were ND and 135 (9.2%) were D. Of the 1330 ND, 1241 (93.3%) were within normal BG range, while 89 (6.7%) were had abnormal BG level. Of the 135 D, 90 (66.7%) were found to be within normal BG range, while 45 (33.3%) had abnormal BG level.
Conclusions: We conclude that the incidental finding of abnormal BG levels among ND is not uncommon (6.7%). When present, such finding not only has potential implications on the quality of the exam but also may affect patient management. Routine measurement of BG level prior to FDG injection should be a standard of care in all patients undergoing PET examinations, including D as well as ND.
