Objectives: The purpose of this study was to evaluate if the whole-body FDG-PET was able to give information in addition to conventional imaging studies, and to make clinical impact on the management in patients with multiple myeloma.
Methods: 33 whole-body FDG PET scans were performed in 15 patients with multiple myeloma between October 2003 and April 2007. These scans were divided into three groups according to purpose: A. 7 scans in 7 pts for staging before therapy, B. 17 scans in 7 pts during therapy, and C. 9 scans in 6 pts after complete therapy. The PET findings were evaluated in terms of additional information to other imaging modalities, such as radiography or CT, and clinical impact for therapeutic management. The laboratory data, histopathological findings, and/or clinical course were used as standard reference.
Results: In Group A, PET results were positive in 2 scans, and negative in 5 scans. Three additional findings, including one focal and one diffuse bone marrow involvement, were obtained in 3 cases (43%); however, there was no clinical impact on management. In Group B, PET demonstrated 8 additional findings in 7 cases (41%). PET findings were helpful in diagnosing residual viable tumors in 2 cases and post therapeutic changes in the bone in 5 cases. The therapeutic strategy was influenced in one case (6%) by changing protocol of chemotherapy. In Group C, PET could detect one recurrent lesion that had not been identified, and chemotherapy started in one case (11%). On the other hand, no additional findings or clinical impact was found in the remaining 8 scans.
Conclusions: FDG-PET could give additional findings to conventional modalities in patients with multiple myeloma, but the number of cases, in which PET affected therapeutic management, was small in this investigation.
