New Approaches Using SPECT, PET and Hybrid Imaging

Experimental study of a new myocardial perfusion imaging agent 99mTc-N-DBODC5 in swine

Objectives: 99mTc-[bis (dimethoxypropylphosphinoethyl) ethoxyethylamine] - [bis (N-ethoxyethyl) dithiocar-bamato] nitride (99mTc-N-DBODC5) is a new monocationic, lipophilic agent. We sought to study the pharmacokinetics and organ biodistribution of in normal swine.

Methods: The average labeling yield of solution of 99mTc-N-DBODC5 was (95.54±0.85)%. Seven anesthetized Chinese mini-pigs were injected from the ear vein with 247-306 MBq 99mTc-N-DBODC5. Samples of blood were collected from the femoral vein at 2, 5, 10, 15, 30, 45, 60, 75, 90, 120, 150 and 180 min following the injection of 99mTc-N-DBODC5, serial plane images were acquired meanwhile.

Results: 99mTc-N-DBODC5 cleared rapidly from the blood pool. The blood disappearance of 99mTc-N-DBODC5 was biexponential with an initial half-time of 2.97±0.48 min and a late half-time of 52.49±19.49 min, calculated blood clearance was 0.24±0.14 L/min. Myocardial 99mTc-N-DBODC5 uptake was similar to 99mTc-MIBI, and there was no significant clearance over 180 min. Both tracers exhibited minimal lung uptake. Importantly, 99mTc-N-DBODC5 cleared more rapidly from the liver than 99mTc-MIBI. As early as 30 min after injection, 99mTc-N-DBODC5 heart-to-liver ratio was significantly higher than 99mTc-MIBI (0.99±0.03 vs. 0.46±0.10, P<0.05), and the difference further widened by 90 min (2.18±0.14 vs. 0.69±0.11, P<0.01).

Conclusions: The rapid 99mTc-N-DBODC5 liver clearance may shorten the duration of imaging protocols by allowing earlier image acquisition and may markedly reduce the problem of photon scatter from the liver into the inferoapical wall. So 99mTc-N-DBODC5 is a promising new myocardial perfusion tracer worth further investigation.
