Objectives: Aim was to establish the operation of 68Germanium(T
=270.8 days)/68Gallium(T
=68min) generator and synthesize 68Ga-DOTATOC to be used for the diagnosis of NETs expressing somatostatin receptor SSTR2. Quality controls were also done before injection.
Methods: 68Ge/68Ga generator(Cyclotron Co.,Obninsk, Russia) was eluted with 7-10ml of 0.1NHCl to obtain 68Gallium. Impurities of 68Ge(IV), Fe(III), Zn(II) and Ti(IV) from eluate were removed using 1.0ml solution of acetone+HCl and concentrated on an cation ion exchanger using 400µl solution of acetone+HCl. Radiolabelling of DOTATOC with 68Ga was carried out in a reaction vial at 127°C for 10minutes. Preparation was purified on Sep-Pak C18 cartridge to remove the unlabelled peptide and 68Ga followed by recovery of 68Ga-DOTATOC with 400µl ethanol in a sterile vial through a 0.22micron filter. Quality control was performed using TLC plates.
Results: Preparation was a clear solution having pH 6.5-7.0 and Rf=0. 68Ge content was negligible. RCP was observed to be >99% with a specific activity of about 10-12MBq /µg peptide. Yield was 50-60%.Total time taken in preparation was just 20 minutes.
Conclusions: 68Ga can be eluted from 68Ge/68Ga generator in purified form and 68Ga-DOTATOC can be synthesized easily within 20 minutes. Generator can be eluted 2-3 times in a day. PET imaging has been successfully done and established the diagnosis like duodenal carcinoid, insulinoma, gastrinoma etc.68Ga-DOTATOC is a potential PET tracer for imaging NETs.
