Extremity radiation protection of the staff in 90Y-DOTATOC therapy

Objectives: Evaluation of the staff radiation extremity exposure from preparation and application of 90Y-DOTATOC in our hospital.

Methods: 15 therapeutic administrations were considered. The chemists performed the radiolabelling at a high protection standard in a dedicated hot cell, using Plexiglas shieldings, tele-tongs and an automatic dispensing system (MASPRES, Firenze, Italy). Physicians administered the radiopharmaceutical through a slow infusion system lasting 15-25 min per patient. Operators wore latex and anti-X gloves. Thin-layer TLDs (LiF:Mg,Cu,P) were fixed to the fingertips to measure the local skin dose, Hp(0.07).10 TLDs were given to the chemist, while 6 to the physician.

Results: As for the chemist fingertips, the 75th percentile of the dose distribution is 0.222 mSv/GBq, while the 95th is 1.640 mSv/GBq. The averaged absorbed doses during single radiolabelling procedures have a mean value (s.e.) of 0.400 (0.196) mSv/GBq , which ranges from 0.015 to 3.012 mSv/GBq, and median 0.104 mSv/GBq. The ratio between the highest and the lowest absorbed dose to the fingertips is extremely variable, observing values from 4.6 to 69.4. The physician averaged doses range from 0.002 to 2.725 mSv/GBq (max-to-min-value ratio of 1181) with mean value (s.e.) 0.432 (0.196) mSv/GBq. The right thumb received the highest dose (0.934 mSv/GBq) of both hands, while the left middle finger the highest dose of the left hand (0.553 mSv/GBq). No significant difference (p=0.29) was observed between right and left hand (respectively mean value of 0.483 and 0.303 mSv/GBq).

Conclusions: As accidents may easily happen in this kind of manipulation, working at high protection level is really recommended.
