Characterization of unclear findings at FDG-PET/CT in cancer pts

Objectives: FDGPET is a sensitive technique for the evaluation of cancer patients.However,a major issue is the low specificity as the differential diagnosis between cancer and aspecific uptake is not always possible.Aim of our study was to determine the incidence of unclear findings in the clinical practice and evaluate the contribute of dual time PET to the final diagnosis.

Methods: Our population consisted of 180consecutive pts referred to our Centre for cancer in 10working days.Each pt underwent a FDG PET/CT according to the standard procedure.The images were evaluated by 2nucl med doc aware of the clinical history.In case of focal unclear findings a second acquisition was performed.SUVmax was measured in both the acquisitions.Mean uptake times were 55 and 85min.

Results: 10/180 pts(5F,5M,age 68)showed unclear findings(mean SUVmax 9,2)in the bowel(6pts),in the lung(2pts)and in the H&N(2pts).5pts had NHL,3lung ca,1HD,1colon ca.In 4/10 patients(2lung and 2bowel) the SUVmax dropped on average of 4,5 and in 1/4 the uptake disappeared.All these pts were reported as negative and this was confirmed by the follow-up.In 6/10 pts(4bowel and 2H&N)the SUVmax was stable(2pts) or increased(4pts) on average of 2,5. These findings were reported as suspicious for cancer and the pts are under further evaluations.

Conclusions: In our experience the dual time PET contributed to clarify the diagnosis in 4/180 pts(2%)in whom the SUV dropped,while in 6/180 pts further investigations were suggested and the diagnostic accuracy of PET did not change.Although in a minimal percentage of pts,the dual time PET may be considered a useful tool in the clinical practice.
