Bone metabolism and leptin in HIV patients on HAART

Objectives: The data regarding the relationship between bone metabolism and leptin are inconsistent and conflicting.We investigated potential changes in serum leptin correlated to bone mineral density (BMD) in HIV pts on HAART who can develop osteopenia and even osteoporosis as long term complications.

Methods: In 117 HIV HAART pts (67 M,50 F) and 50 normal controls (23 M,27 F) all with normal body mass index (BMI),we measured whole body BMD by DEXA,subdividing pts with low (L: < -1) or normal (N: ≥ -1) T-score,total body fat (TFM) and percent fat (F%),in serum leptin,osteocalcin (OC),bone alkaline phosphatase (BAP) and 1,25 (OH)2 D,and in urine pyridinium cross-links (PYD & DPD).

Results: 54/117 (46.1%) pts were L,63/117 (53.9%) N;32/54 L and 35/63 N pts were males and the other females.BMD was significantly (p<0.01) lower in L than in N in both sexes;25/32 L males and 19/22 females were osteopenic,the remaining osteoporotic. Mean TFM,F%,OC,BAP and PYD & DPD were higher, the latter two significantly (p<0.02), and 1,25 (OH)2 D lower in L than in N, as was significantly higher mean leptin for males (p<0.002) and females (p<0.03). Leptin positively correlated with TFM, F%, BAP and PYD & DPD in both sexes, while leptin, TFM and F% negatively correlated with BMD as did 1,25 (OH)2 D with PYD & DPD in females.In 56 rechecked HIV patients continuing HAART, leptin and BAP increased and 1,25 (OH)2 D decreased,but not significantly;BMD significantly decreased in females and PYD & DPD increased in males (p<0.02).

Conclusions: An inverse relationship was found between leptin and BMD in HIV HAART pts with osteopenia/osteoporosis.In this condition, leptin role on bone metabolism is still unclear although an inhibitory effect can be hypothesized in combination with HAART.
