Alteration of striatal dopamine 2/3 receptor binding in patients with idiopathic generalized epilepsy

Objectives: The dopaminergic system is supposed to be involved in controlling epileptic activity in focal epilepsias. The objective was to quantify interictal extrastriatal and striatal D2/D3 receptor binding in patients with idiopathic generalized epilepsy (IGE) using the high affinity dopamine D2/D3 receptor PET-ligand 18F-Fallypride.

Methods: Eight IGE patients (5 m, 3 f) and 21 age matched control subjects (21 m) were studied. Dynamic images (180 minutes) were acquired after injection of 172-180 MBq 18F-Fallypride. Parametric images of binding potential (BP) were created using the simplified reference tissue model. The images were stereotactically normalized using a ligand-specific template. We performed a voxel-based analysis with statistical parametric mapping (SPM2). A subsequent ROI analysis was done comparing the BP of caudate nucleus , putamen and ventral striatum.

Results: IGE patients showed a significant decrease in 18F-Fallypride BP compared to controls (SPM analysis p<0.001 corr. at cluster level) in the bilateral caudate nucleus, putamen and ventral striatum. There was no significant alteration of 18F-Fallyprid binding in extrastriatal regions. The significant decreases in ROI analysis (p<0.001) were as following: left caudate nucleus 24% right caudate nucleus 23%, left putamen 28%, right putamen 35%, left ventral striatum 22%, right ventral stiatum 19%.

Conclusions: Patients with IGE show a considerably reduced D2/3 receptor binding in the bilateral caudate nucleus, putamen and ventral striatum. This suggests a specific alteration of the dopaminergic system in the striatum of IGE patients similar to the known reduced BP of the temporal lobe of patients suffering from mesial temporal lobe epilepsy.
