Objectives: MELAS (Mitochondrial Encephalomyopathy, Lactic Acidosis, and Stroke-like episodes) is a disease caused by mutation of mitochondrial DNA. Metabolism in mitochondria and cardiac energy efficiency may change in patients of MELAS. Oxidative metabolism and myocardial efficiency in MELAS and normal subjects were compared using C-11 acetate PET.
Methods: Five MELAS patients and 6 normal volunteers were analyzed. All the subjects underwent C-11 acetate dynamic PET at rest. Washout rate constant (Kmono) was measured as an index of oxidative metabolism. Double-product corrected Kmono (cKmono) and work-metabolic index (WMI) were also calculated.
Results: MELAS showed significantly lower cKmono value at rest. However, when WMI which is an index of cardiac efficiency was compared, MELAS showed no significant difference; even tend to show higher WMI value than normal.

Conclusions: MELAS tends to show similar or rather slightly higher WMI than normal subjects. It is very unlikely to patient with MELAS have similar or even better cardiac efficiency than normal subjects, thus this finding could indicate that washout of C-11 acetate is uncoupled with myocardial metabolism in specific conditions that affect on the mitochondrial metabolic chain such as MELAS.
