Annexin V compared to octreotide scintigraphy for the diagnosis of autoimmune myocarditis in the rat

Objectives: 99mTc-Annexin-V (ANX) is a marker of cell death. 111In-octreotide (OCT) is a marker of somatostatin receptors overexpressed on inflammatory cells. We aimed to compare ANX and OCT for scintigraphic detection of experimental autoimmune myocarditis.

Methods: Acute myocarditis was induced in rats by injection in hind footpad of rat purified cardiac myosin in complete Freund’s adjuvant on day 1 and 7. Control rats were injected only with Freud's adjuvant. On day 21, the rats received iv injection of ANX (7,4 MBq/100g) or OCT (3,7 MBq/100g) then 4h later, 1h anterior thoracic images were obtained using a small animal gamma-camera. Heart-to-lung activity ratios (HLR) were calculated on the scintigrams. After imaging, quantitative autoradiography and histological studies of myocardial sections were performed. Apoptic cells (apostain), macrophages (ED1), and lymphocytes (CD4 and CD8) were revealed using immunological techniques.

Results: On imaging, significant ANX uptake was seen in cardiac area of all 11 immunized rats, with higher HLRs than in 5 controls (4.83 ± 1.30 vs 3.12 ± 0.4, P < 0.001). The 6 immunized rats injected with OCT also showed higher HLRs than 4 controls (3.09 ± 0.83 vs 2.18 ± 0.31, P < 0.05). Autoradiographic studies showed significant heterogeneous myocardial ANX and OCT uptake correlated to the degree of cell death and inflammation on histology, with 223+/-181 mean counts/mm2 vs 13+/-4 in controls for ANX (p=0.04), 77+/-39 versus 8+/-2 in controls for OCT (p=0.02).

Conclusions: Rats with autoimmune myocarditis showed higher ANX than OCT uptake in myocardial lesions. This confirms potential role of ANX imaging for non invasive diagnosis of acute myocarditis in patients.
