Reproducibility of deep inspiration breath-hold 18F-FDG PET/CT technique in diagnosing cancer located in the area affected by respiratory motion. Evaluation of SUVmax and metabolic volume

Objectives: Deep-inspiration breath-hold (DIBH) PET/CT is useful method for diagnosing various cancers located in the area influenced by respiratory motion. However, reproducibility of measuring the maximum SUV (SUVmax) and the metabolic volume (MV) of the tumor has not been validated. We evaluated the reproducibility of measuring both SUVmax and MV in DIBH image.

Methods: Fifty patients with various cancers (27 thoracic lesions and 23 abdominal lesions) were enrolled. 50 minutes after 185 MBq 18F-FDG intravenous injections, whole body (WB) imaging was done. According to the findings of WB image, we added the DIBH spot image 30 min after the early image. Each DIBH spot imaging was done 4 times with acquisition time of 30 seconds respectively. We calculated the coefficient of variation (CV) of both SUVmax and MV in DIBH images of each patient. Finally, we calculated mean values of CV in both indices in all patients.

Results: The mean value of CV of SUVmax in the thoracic lesions was 6.4 and the mean value of CV of SUVmax in abdominal lesions was 7.5 respectively. Similarly, The mean value of CV of MV in thoracic lesions was 9.6 and that in abdominal lesions was 10.7 respectively.

Conclusions: In evaluating cancer which locates in the area affected by respiratory motion, reproducibility of 18F-FDG PET/CT by DIBH technique for measuring both SUVmax and MV is fairly good. However, we should take into account that the value of CV in abdominal lesions is relatively higher than that in thoracic lesions.
