Objectives: 8H9 is a murine IgG1 monoclonal antibody that targets 4Ig-B7H3, an antigen expressed on neuroblastoma and solid tumors. PET/CT imaging with 124I-8H9 provides higher quality images for accurate biodistribution and dosimetry assessment.
Methods: Five patients injected with intrathecal 124I-8H9 were imaged serially using PET/CT at approximately 4, 24, and 48 hours post-injection. Patients had CNS tumors 1.7 to 2 mCi of 124I-8H9 was given intrathecally through an Ommaya shunt. Serial cerebrospinal fluid (CSF) samples (typically 4-5) were obtained. Images were analyzed by placing regions of interest on the spinal canal at all three time points. PET images provided direct activity measurements within the CSF compartment was used to perform dose estimates to the CSF.
Results: 124I-8H9 PET scans provided high resolution images of the antibody distribution with lesion targeting visible in 3 patients with structural lesions on MRI. At 24 hours, much antibody had cleared from the ventricles and was seen in the spinal canal and around the cerebral convexities. The distribution corresponded well to 111In- DTPA cisternography. The biological T1/2 clearance ranged from 8.9 hr to 64.6 hr with corresponding doses of 14.1 to 92.9 cGy/mCi to CSF.
Conclusions: 124I-8H9 PET/CT provides high resolution quantitative images of targeting and for dosimetry. Future work will compare the PET activity values with CSF sample data.
