Risk stratification using dipyridamole thallium-201 SPECT myocardial perfusion imaging with ECG gating in asymptomatic pilots with latent coronary artery disease

Objectives: To evaluat the diagnostic value of risk stratification using dipyridamole thallium-201 SPECT myocardial perfusion imaging with ECG gating in asymptomatic (flight) pilots whose exercise treadmill test were positive.

Methods: One hundred and twenty asymptomatic pilots whose exercise treadmill test results were positive underwent dipyridamole stress and thallium-201 myocardial perfusion imaging with ECG gating from the year 1998 to 2005. The stress and three hours later redistribution imaging were analysed by three nuclear medicine physicians together. The quantitative software of Emory Cardiac Toolbox was used.Those pilots were divided into positive or negative group according to the results of myocardial perfusion imaging. Over a mean follow-up period of 6.5 years(from 1 year to 8 year),the rate of MACE(major cardiac events) and death between the two groups were compared.

Results: The final results of myocardial perfusion imaging showed that 48 pilots were positive and 72 pilots were negative. MACE was found 31%(15/48) pilots in the positive group and 4%(3/72) pilots in the negative group subjects,the difference was significant( p< 0.01). Only two cardiac death cases was found in positive group and no cardiac death was found in the negtive group. The pilots with two or more segments ischemia in imaging were apt to developing cardiac events compared with negative groups (p<0.05).

Conclusions: Dipyridamole thallium-201 SPECT myocardial perfusion imaging with ECG gating in asymptomatic pilots with latent coronary artery disease is much more accuracy to detect myocardial ischemia in ealy stage than exercise treadmill test, and is also very helpful in predicting cardiac event.
